Overview

Many individuals engage in behaviours which negatively affect their physical and mental health (e.g., gambling, drinking, smoking) and find it difficult to change these behaviour patterns without specific goals which they can work towards. This worksheet can be used to help a client monitor their progress in achieving the goals they have set with their psychologist.
